Benefits at St. Paul Heart Clinic
Below is a summary of the 2010 benefits that are currently offered at St. Paul Heart Clinic:
Benefit eligibility with 20 hours/week. Benefit costs are based on employment status with full-time equal to 32 hours/week .
Health insurance: Preferred One open access with one plan option and two sources of funding: Plan option is is a high deductible with employee selecting one of these two sources to partially fund the deductible: Health savings account (HSA) employer contribution and a voluntary employee contribution option, or Health reimbursement account (HRA) employer contribution option Employee selects the source of funding.
Dental insurance: Humana Dental PPO
Life insurance: One and one-half your hourly rate x 2080 hours to a maximum of $50,000 of coverage. Dependent life insurance: $5,000 for a spouse and $2,000 for each dependent child.
Disability insurance: Short term disability pays at 50% of your gross salary each day beginning at the 11th working day for the next 20 working days. Long term disability pays at 60% of your gross salary each day beginning at the 90th calendar day of a disability.
Pretax benefits: Premium conversion, parking, healthcare spending and dependent care options. A limited healthcare spending account is available for employees who participate in the high deductible, health savings account (HSA) health insurance option.
Voluntary benefits: Available for purchase: Additional life, short term disability, hospitalization, accident and cancer insurance.
Profit Sharing Plan: Discretionary profit sharing contribution based on a percentage of earned compensation. Eligible after one year of employment and 1000 hours worked. Enrollment dates are January 1 or July 1 of each year after satisfying the eligibility requirements.
401K: Employee pretax deferral up to 50% of earned compensation to maximum defined by the IRS. Eligibility requirements are the same as for the Profit Sharing Plan.
Paid Time Off (PTO): 16 days each years one and two for full-time. One day incremental increases from year three to year five. This rate may vary by department. Human Resources will notify you if the above is not your accrual.
Parking: Available, approximate monthly employee cost is $70.00. No charge for parking at the Maplewood and Lakeview offices. Parking costs are payroll deducted on a pretax basis. Discounted transit passes are available for purchase on a pretax basis.
Paid Holidays: Approximately nine each calendar year.
Professional Development: Benefit amount varies by department. Professional Development must be applicable to the employee’s position and be approved by the director/supervisor.
Reimbursements: Cell phone usage and mileage for business travel.
