Insurance participation
St. Paul Heart Clinic in Minnesota participates in most major health plans. We recommend that you verify with your insurance company that we are participating providers for your specific policy. Please remember that your insurance policy is a contract between you and your insurance carrier.
We ask that you provide us with complete and accurate current insurance information before receiving services. St. Paul Heart Clinic will file your insurance claims on your behalf, with the understanding that you have full responsibility for payment of any charges not covered by insurance.
Referrals
If your insurance plan requires a referral for you to see us, you are responsible for making sure a referral is in place before you receive services. We will assist you in obtaining the referral from your primary care physician.
Payments due at time of service
Please bring your current insurance cards with you to each appointment. You will be required to pay any copayments at the time of service. If applicable, you may be asked to make a payment toward your insurance deductible at the time of service. If your account balance is past due, payment may be required before your next visit.
Billing statements
Payment in full is expected on or before the due date shown on your billing statement. Please contact us as soon as possible after receiving your billing statement if immediate payment in full would cause serious financial hardship. We’ll be glad to discuss payment arrangements with you. For your convenience, St. Paul Heart Clinic in Minnesota accepts VISA, MasterCard, and Discover.
